Vital Records 1-724-656-3100. For a certified copy of a birth or death certificate, you need to pay a minimal fee and meet the following requirements: be at least 18 years of age and an immediate family member to request a birth certificate. For a death certificate, you must be directly related to the deceased or a legal representative of the deceased's estate.
